Tummy Tuck in Turkey Patient Guide: Abdominoplasty and Diastasis Repair

A Safe, Functional and Aesthetic Transformation That Restores the Body’s Anatomical Integrity

> *"The deformities that develop in the abdomen after pregnancy or significant weight changes are not only an aesthetic concern; they are an anatomical condition that disrupts the biomechanical balance of the anterior abdominal wall. Our surgical approach is not just to remove sagging skin, but to rebuild the inner muscle wall like an internal corset, restoring the body's core balance and natural silhouette."*
>
> **— Assoc. Prof. Dr. Ozan Balık, Specialist in Plastic, Reconstructive and Aesthetic Surgery**

After pregnancy, after significant weight loss or as the body changes over the years, the skin and muscle structure of the abdomen can change too. Exercise and healthy habits help strengthen the abdominal muscles; however, they cannot always remove loose skin or the gap between the abdominal muscles. For some people, the abdomen they see in the mirror may no longer match the way they feel good about themselves.

A tummy tuck is one of the options people experiencing these changes can consider. The aim is not to give everyone the same abdomen, but to address the excess skin and bring the abdomen into better harmony with the rest of the body. If the abdominal muscles have separated, repairing this is also added to the surgical plan in suitable patients.

In our clinic, in almost all tummy tuck operations, depending on the suitability assessment, **the fascia of the rectus abdominis muscles is plicated and diastasis repair is also performed** . This aims to address the separation in the anterior abdominal wall and achieve a firmer abdominal contour. The extent of the repair is planned according to the examination and surgical findings.

For patients travelling to Istanbul from abroad for surgery, the process itself also matters. Discussing every step in advance, from the online pre-consultation to the in-person examination and from the tests to the post-operative check-ups, helps the patient know what to expect. In this guide we explain the types of tummy tuck, diastasis repair, the recovery period and the treatment process in Istanbul.

## What is a tummy tuck?

A tummy tuck, medically called **abdominoplasty** , is an operation that aims to remove excess skin from the abdomen and reshape the abdominal contour. Depending on the patient's needs, a procedure on the fatty tissue and repair of the abdominal muscles may also be added to the plan.

Loose abdominal skin can develop for different reasons, such as pregnancy, significant weight loss, ageing and individual tissue characteristics. After pregnancies or weight changes, the abdominal muscles may also move apart in the midline. This is called **diastasis** . In some people, this separation of the anterior abdominal wall can contribute to the abdomen looking protruding or loose.

Diastasis repair does not mean that the abdominal muscles themselves are cut and shortened. The fascia, the connective tissue in the midline of the anterior abdominal wall, is assessed. In suitable patients this fascia is brought together in the midline with sutures and plicated; the aim is to repair the gap between the rectus abdominis muscles. Whether the repair is suitable for the patient and its extent are determined at the examination.

A tummy tuck is not a weight-loss method. NHS patient information also states that the operation is not used for weight loss; it aims to address excess skin and fat in the abdomen and, in some patients, muscle laxity.

## Diastasis repair: what does plication of the fascia mean?

The rectus abdominis muscles run down the front of the abdomen on the right and left. The fascia that holds these muscles together in the midline can stretch and widen after pregnancy or significant weight changes. When the distance between the two muscles increases, this is called diastasis.

In diastasis repair, in suitable patients, this fascia can be brought together in the midline and plicated with sutures. Assessing this aspect of the abdominal wall at the same time as addressing the excess skin in a tummy tuck allows a more complete plan. In our clinic, diastasis repair with fascial plication is performed in almost every tummy tuck; the individual approach is determined by the examination and surgical findings.

Patients should not expect this procedure to make the abdominal muscles "stronger" or to achieve weight loss on its own. The goal is to repair the separation in the anterior abdominal wall and firm up the abdominal contour. When to return to movements that put strain on the abdomen after surgery is decided by the surgeon according to your recovery.

## Mini tummy tuck or full tummy tuck?

"Tummy tuck" is not a single surgical plan. The location and amount of laxity in the abdomen, the position of the belly button, the condition of the muscles and the patient's expectations determine which approach is considered.

**A mini tummy tuck** can be considered in suitable patients in whom the excess skin and laxity are mainly below the belly button. It should be remembered that this approach does not mean an easier or shorter operation for every patient. If there is diastasis of the abdominal muscles, the need for repair is also assessed separately in planning.

**A full tummy tuck** can address a larger area. Where necessary, the skin around the belly button is repositioned and the fascia of the abdominal muscles is plicated to repair the diastasis. The incision is usually planned in the lower abdomen so that it can stay under underwear or swimwear. Still, the incision scar is permanent; its appearance can change over time, but it cannot be said to disappear completely.

In some patients a tummy tuck can be planned together with liposuction. This decision is made after assessing body structure and health. Adding several procedures to the same operation can change the operating time and recovery needs.

## Who can be considered for a tummy tuck?

A tummy tuck can be considered for people whose general health is suitable and who are bothered by excess skin, laxity or diastasis in the abdomen. In people who are close to their weight goals and whose weight has been stable for some time, the surgical plan can be made more soundly.

Future pregnancy plans are one of the topics that need to be discussed when deciding on surgery. Because pregnancy can stretch the abdominal skin and muscles again, it can affect the results of the operation. Similarly, the plan of patients who have recently experienced significant weight change is assessed separately.

At the examination, general health, medications, previous operations, smoking or nicotine use, weight and pregnancy history, the condition of the abdominal skin and muscles, diastasis findings and expectations are discussed. A history of blood clots or circulatory problems must also be shared.

## The first step: a free online consultation

For patients abroad considering coming to Istanbul, the process can start with a free online pre-consultation. In the consultation you can describe the changes in your abdomen, what bothers you and what kind of result you would like. Your medical and surgical history is also discussed for the initial assessment.

At the first stage, photos or videos from certain angles may be requested. These images help with a preliminary assessment of the general contour of the abdomen and the possible surgical approach. However, they do not replace a real examination. The elasticity of the skin, the condition of the abdominal wall and the diastasis can be assessed more reliably at an in-person examination.

The purpose of the online consultation is not to promise a definite result, but to start the process and answer your questions. It is important to share your medications, supplements, allergies and previous operations. Preparing your questions in advance also makes the consultation more productive.

## How is the surgical plan made?

After you arrive in Istanbul, an in-person examination is carried out. The abdominal skin, fat distribution, abdominal wall and diastasis findings are assessed. The expectations you stated in the pre-consultation are discussed again; the initial plan is updated according to the actual examination findings.

The necessary blood tests and anaesthesia assessment are then planned. The results and your medical history are reviewed together to assess your suitability for surgery. At this stage it may be necessary to change the surgical plan or postpone the procedure. This is a natural part of assessing the patient correctly.

At the final consultation before surgery, the incision site, removal of excess skin, diastasis repair by plication of the rectus abdominis fascia, the approach to the belly button and the recovery process are discussed. It is also necessary at this stage to consider the limits of the operation and the fact that there will be a scar.

## What happens during the operation?

A tummy tuck is usually performed under general anaesthesia. The technical details of the surgical approach vary according to the type of operation planned for the patient. In suitable patients, excess skin is removed, the abdomen is reshaped and the fascia of the rectus abdominis muscles is plicated in the midline to repair the diastasis. In patients where it is considered necessary, a procedure on the fatty tissue may also be planned.

After the operation the patient is kept under observation in hospital. The length of the hospital stay depends on the scope of the operation and the patient's clinical condition. The decision to discharge is made after the post-operative assessment.

## The treatment process in Istanbul

For patients coming from abroad, planning the travel, accommodation and surgical steps together can reduce uncertainty. The general flow can be as follows:

1. **First contact and online consultation:** Your expectations and medical history are discussed; photos or videos may be requested for the initial assessment.
2. **Travel and accommodation plan:** Your flight dates and length of stay in Istanbul are confirmed. Organisational support can be provided with hotel options to the standards you request.
3. **Arrival in Istanbul:** Your transport to the hotel or clinic can be arranged with a planned VIP transfer.
4. **In-person examination and tests:** Your abdomen, abdominal wall and diastasis findings are assessed; the necessary tests and the anaesthesia consultation are carried out.
5. **Final pre-operative consultation:** The surgical plan and post-operative care details are discussed again.
6. **Surgery and hospital monitoring:** After the procedure you are observed in hospital according to your clinical condition.
7. **Hotel and check-ups:** When appropriate, you can move to your hotel with a VIP transfer. The planned check-ups take place during your stay in Istanbul.
8. **Return and follow-up:** Your post-operative condition is assessed before your journey home. Online contact continues after you return home.

Health insurance options and coverage can also be discussed in advance. The dates on which the policy is valid, what it covers and its exclusions are stated in the insurance document. You need to review the conditions before travelling.

## How long do you need to stay in Istanbul?

After a tummy tuck, the patient's stay in Istanbul should be planned to cover the surgeon's check-ups and the assessment before the return. In our clinic, **the general plan for a tummy tuck is usually a 7–10 day stay in Istanbul** . The scope of the operation, the course of recovery, the use of drains and the flight time can affect this plan.

When planning your flight ticket, it is therefore helpful to make sure the return date can be changed and to allow for the possibility of a longer stay if necessary. Fitness for the return flight is determined by assessing your post-operative condition.

## What can you expect during recovery?

In the first days, pain, tightness, swelling and bruising may occur in the abdomen. When diastasis repair has been done, tightness may be felt in the abdominal wall. After a full tummy tuck, standing upright may be difficult at first; this usually eases as your body heals. Temporary changes in sensation or numbness may also occur.

After the operation the patient may be given a special compression garment. The garment is used to support the post-operative care plan and help control swelling. You will be told how long and how to wear it. The garment should not be excessively tight or cause marked discomfort.

Drains may be used in some patients. Drain care, emptying and when they will be removed are assessed at the check-ups. Not every patient needs drains for the same length of time, or at all.

Keeping the knees slightly bent when lying down can help reduce tension in the abdomen. You will be given individual instructions on short walks and movement. Returning to work, driving and exercise is planned according to the scope of the procedure and your recovery.

## The scar and the settling of the result

After a tummy tuck there is an incision scar in the lower abdomen. In a full tummy tuck there may also be a scar around the belly button. The incision is planned as far as possible so that it can stay under underwear or swimwear; still, the length and appearance of the scar vary with the person and the scope of the operation.

In the early period the scar may be more visible, and its appearance can change over time. It cannot be said to disappear completely. Follow your surgeon's recommendations on wound care and scar management.

It takes time for the swelling to go down and the tissues to settle. The appearance immediately after the operation does not show the final result. The result can be affected not only by recovery but also by weight changes, pregnancy and individual tissue characteristics.

## An open conversation about possible risks

As with a tummy tuck, the risks of any surgical procedure that includes diastasis repair should be discussed before deciding. These can include infection, bleeding, delayed wound healing, fluid collection, changes in sensation, a noticeable scar, contour differences or dissatisfaction with the result. Less common but important risks include blood clots, lung problems and complications related to anaesthesia. In its tummy tuck safety information, the ASPS lists these possibilities and the importance of a detailed assessment with one's surgeon.

The purpose of discussing these topics is not to worry the patient but to make all aspects of the decision understandable. Your personal level of risk is determined by assessing your health, the scope of the operation, nicotine use, mobility and other factors.

If after the operation you have rapidly increasing pain, fever, unexpected swelling, marked redness or discharge at the wound, shortness of breath or chest pain, contact the medical team without delay. For emergency symptoms, go to your local emergency medical service.

## Is our surgery covered by health insurance?

Within the health protection programme for international patients, our clinic will arrange your health insurance and complication insurance before you even arrive in Turkey. This way, from before you board your plane until six months after you return to your country, you will be under the protective umbrella of our national health system.

## What affects the price of a tummy tuck?

The price can vary according to whether a mini or full tummy tuck is planned, the extent of the diastasis repair, additional procedures, hospital and anaesthesia conditions and the services offered. When getting a quote, it is useful to ask whether the hospital stay, check-ups, transfers and accommodation arrangements are included in the price.

In Turkey, surgeons may set different prices depending on their experience, the hospital where they operate, the materials used and the scope of service. Our clinic strives to offer high-quality care at accessible prices while treating patient safety as the top priority. Price alone is not a sufficient criterion for choosing a surgeon or hospital; the surgical plan and the follow-up process should also be assessed.

## Frequently asked questions

### Is diastasis repair performed in every tummy tuck?

In our clinic, in almost every tummy tuck, depending on the suitability assessment, the fascia of the rectus abdominis muscles is plicated and diastasis repair is performed. The need for and extent of the repair are planned according to the examination and surgical findings.

### What does diastasis repair mean?

The fascia associated with the separation of the abdominal muscles in the midline is, in suitable patients, brought together in the midline with sutures and plicated. The aim is to repair the separation in the anterior abdominal wall. This procedure is not a weight-loss method.

### Is a tummy tuck done to lose weight?

No. A tummy tuck is not a weight-loss method. It aims to address excess skin and contour changes in the abdomen.

### Is there a scar after a tummy tuck?

Yes. There is an incision scar in the lower abdomen; in a full tummy tuck there may also be a scar around the belly button. The appearance of the scars can change over time, but it cannot be guaranteed that they will disappear completely. In any case, these scars stay inside your underwear and usually do not make anyone unhappy. If desired, a tattoo can be done over them.

### When can I return to work and exercise after surgery?

This depends on the scope of the operation, the diastasis repair and the physical demands of your job. Decide your timeline for returning to activities with your surgeon.

### How long do I need to stay in Istanbul?

Our general plan usually covers **a 7–10 day stay** . However, this period does not mean clearance to fly for everyone. The timing of your return is decided taking into account the post-operative assessment and the flight time; a longer stay may be recommended if necessary.

### Does follow-up continue after I return home?

Yes. After you leave Istanbul, your recovery can be followed through online consultations. If you have new or urgent symptoms, you need to get medical care where you are.

## Follow-up after you return home

Recovery after surgery is not complete with a visit of a few days to Istanbul. It takes time for the swelling to go down and the body contour to settle. In our clinic we aim to keep online contact with our patients for at least one year. These consultations help us assess the recovery process and answer the patient's questions.

Online follow-up does not always replace an in-person examination. If a new or unexpected symptom appears, the patient may need to get medical care where they are. Before you return, it should be explained whom you will contact and how, what to do in an emergency and when the check-ups will take place.

In addition, all our international patients are insured under the safety umbrella of the health authority in Turkey, and this insurance will continue to protect you even in your own country.

## A personal tummy tuck plan in Istanbul

If laxity, excess skin or diastasis in your abdomen bothers you, the first step is a consultation in which we can assess your expectations and health together. A free online consultation can help answer your first questions and plan your visit to Istanbul. Then a personalised surgical plan is made with a real examination, tests and an anaesthesia assessment.

In a tummy tuck plan, the abdominal wall is assessed as well as the excess skin. In our clinic, in almost every tummy tuck, the fascia of the rectus abdominis muscles is plicated and diastasis repair is performed. The details of the plan are finalised according to the patient's examination findings.

For patients coming from abroad, arranging accommodation and VIP transfers can make the process easier. For a tummy tuck we usually plan a stay of 7–10 days in Istanbul; the timing of the return flight is decided after your post-operative condition has been assessed. Online contact and follow-up continue after you return home.
