BBL in Turkey Patient Guide: A More Aesthetic Buttock Contour with Your Own Fat

A Natural, Safe and Personalised Transformation Shaped by Your Own Tissue

> *"Our relationship with our body is extremely delicate and personal. Years of surgical and academic practice have shown me that a successful aesthetic procedure is not about looking like someone else, but about safely reaching the best version of yourself within your own anatomical balance."*
>
> **— Assoc. Prof. Dr. Ozan Balık, Specialist in Plastic, Reconstructive and Aesthetic Surgery**

The rhythm of samba, colourful costumes and carnival scenes filling the streets of Rio… For many people, Brazil first brings this lively atmosphere to mind. Over time, the aesthetic ideal associated with Brazil has also attracted interest around the world: a silhouette with a more defined waist, a rounder buttock and body lines that look more in harmony with each other.

BBL is the abbreviation of *Brazilian Butt Lift* . In Turkish it is known as "Brazilian bottom lift". However, the name can be a little misleading: in a BBL, fat taken mostly from the person's own body is transferred to the buttock area; it is not an operation that lifts the buttock by removing excess skin in the classic sense. That will be the subject of another article.

For many people considering a BBL, the goal is not "to look like someone else" but to achieve a more defined contour that is more in harmony with their own body. Some want to increase volume in the upper buttock, some want to soften the transitions at the sides, and some want to better define the ratio between waist and hips. Each of these goals is individual. A good plan assesses the person's anatomy, expectations and safe surgical limits together instead of applying a single beauty standard to everyone.

For patients from abroad considering a BBL in Istanbul, the process often starts with an initial online consultation. Understanding the patient's goals and medical history, answering basic questions before the examination and planning the visit to Istanbul are all part of this first step. In this guide we explain in plain language how a BBL is performed, the role of ultrasound in the safety plan, the recovery period and how the process can work for patients coming from abroad.

## What is a BBL?

In a BBL, fat is removed from suitable areas of the body by liposuction. The fat is prepared for transfer and transferred to the planned areas of the buttocks. The tissue used is the patient's own fat.

Fat can be taken from areas such as the abdomen, waist, back, hips or thighs. Which areas are suitable depends on the person's body structure, fat distribution, skin characteristics and the desired look. The amount to be transferred to the buttocks is likewise individual.

Of course, a BBL plan is not just about the question "how much fat should we add to the buttocks?" The contour of the areas where fat is removed and the transitions around the buttocks also affect the final look. That is why the waist, back, hips and buttocks are assessed together. The aim is to create a shape that is in harmony with the body's natural proportions.

A BBL is not a weight-loss method. Liposuction can shape the contour of certain areas, but it does not replace weight management. Major weight changes after surgery can affect the body lines and the appearance of the transferred fat.

## Could a BBL be right for you?

Suitability for a BBL is not determined only by the wish to increase buttock volume. General health, the amount of usable fat, skin quality, expectations and the ability to follow post-operative care instructions are assessed together.

At the first consultation, your medical history, previous operations, regular medications, allergies and smoking or nicotine use are discussed. It is particularly important to tell us about any history of blood clots, circulatory problems or other health conditions. This information helps determine the most suitable approach for you.

In a patient with limited fat tissue available, it may not be possible to reach the desired volume. Some patients may have excess skin or marked sagging; in that case fat transfer alone may not provide the expected change. In such situations, other options may be considered.

In a good consultation the surgeon explains not only what you want but also what your body allows. A first impression can be gained from photos; however, the final plan takes shape after a real examination and the necessary tests.

## The first step: a free online consultation

For patients considering travelling to Istanbul from abroad, the first step is often a free online consultation. In this conversation you can share your expectations, your medical history and your questions. Photos or videos from certain angles may be requested for the initial assessment.

Photos and videos help to see the body contour and possible fat harvesting areas in a preliminary assessment. Still, they do not replace a real examination. Light, camera angle and pose can change the image; in addition, tissue characteristics and your health cannot be fully assessed from photos alone.

The purpose of this conversation is not to give a definitive result or approval for surgery, but to listen to you and discuss how the process could go. You can describe the look you want and share features you like in other patients' photos. However, another patient's result cannot be guaranteed for you; the plan is made according to your body.

## The role of ultrasound in BBL

When talking about BBL safety, one of the most important topics is which tissue plane the fat is transferred into. There are important blood vessels deep in the buttock area. Transferring fat into the muscle or deep tissues can lead to serious complications. For this reason, current safety recommendations emphasise transferring fat into the tissue plane under the skin, not into the muscle.

Real-time ultrasound can help monitor the position of the cannula and the anatomical plane into which the fat is transferred during surgery. A joint safety advisory published in 2022 by professional organisations including the ASPS, The Aesthetic Society and ASERF supports the use of ultrasound to confirm the safe anatomical plane. [2022 BBL safety advisory of the professional organisations](https://www.plasticsurgery.org/documents/Patient-Safety/BBL/Gluteal-Fat-Grafting-Safety-Advisory_Aug22.pdf)

Ultrasound is a supporting method that gives the surgical team an image. It is an important part of safety; however, it cannot be said to eliminate all risks on its own. The surgeon's experience, correct anatomical planning, patient selection, the operating environment and post-operative follow-up all matter as well. A 2024 study examining ultrasound-guided gluteal fat transfer reported favourable safety results; nevertheless, the results of a single study do not guarantee the same outcome for every patient and every surgical practice. [2024 study: ultrasound-guided gluteal fat transfer](https://pubmed.ncbi.nlm.nih.gov/38124368/)

Patients considering a BBL may find it helpful to ask their surgeon not only "Is ultrasound used?" but also "Which tissue plane is the fat transferred into, and how is ultrasound used during the operation?" This conversation helps the patient understand the surgical plan better.

## How is the operation planned?

The online consultation before you come to Istanbul helps create the initial plan. At the in-person examination your body structure is reassessed; the areas being considered for fat removal and the desired buttock contour are discussed. The patient's expectations, the amount of usable fat and the safe surgical limits are considered together.

Before surgery, the necessary blood tests and anaesthesia assessment are carried out. Suitability for surgery is assessed according to the test results and your medical history. At this stage some details of the surgical plan may change; this is a natural part of assessing the patient and their health correctly.

At the final consultation before surgery, the plan is reviewed once more. The markings on the body, the areas being considered for fat removal and which areas of the buttocks will be targeted are discussed. It is a good time to ask any questions you have.

## What happens during the operation?

Fat is removed from suitable areas by liposuction. The fat is prepared for transfer and transferred to the buttocks in a controlled way. Where the fat is taken from and how much is transferred are determined by the patient's anatomy and the surgical plan.

A BBL can take about two to three hours. However, this time varies with the number of areas from which fat is removed, their volume and the scope of the procedure. The exact duration can be estimated better after the patient has been assessed.

The operation is performed under general anaesthesia. At the pre-anaesthesia assessment, your health, medications and any previous experiences with anaesthesia are reviewed. It is important that you share this information fully.

How long you stay in hospital after the operation depends on your clinical condition and the scope of the procedure. The decision to discharge is made after the patient's post-operative assessment.

## The surgical process in Istanbul

For patients coming from abroad, discussing the different steps of the surgical process in advance helps to plan the trip more comfortably. In general, the process can go as follows:

1. **First contact and online consultation:** Your goals and medical history are discussed; if appropriate, photos or videos are requested for the initial assessment.
2. **Travel planning:** As the surgical plan progresses, flight dates, length of stay in Istanbul and accommodation options are discussed.
3. **Arrival in Istanbul:** A planned VIP transfer can take you to your hotel or the clinic.
4. **In-person examination and tests:** The surgical plan is finalised with the examination, tests and anaesthesia assessment.
5. **Final pre-operative consultation:** The plan and markings are reviewed and questions are answered.
6. **Surgery and hospital monitoring:** After the operation you are monitored in hospital; the length of stay depends on your clinical condition.
7. **Hotel and check-ups:** When appropriate, you can move to your hotel with a VIP transfer and attend your planned check-ups.
8. **Return home and follow-up:** Your surgeon assesses you before your return; online follow-up continues after you get home.

Accommodation can be arranged to the standards the patient wishes. Travel and insurance details can also be discussed in advance. What the insurance covers, its validity dates and exclusions depend on the policy; the coverage should be reviewed before travelling.

## Is a stay of four or five days in Istanbul enough?

This is one of the questions patients ask most often. A stay of four to five days in Turkey, including the day of surgery, is usually enough for a standard BBL. Of course, a patient's medical history can change the length of stay. That is why it is very important to share your medical history with your doctor.

## The recovery period and the BBL pillow

After the operation, swelling, bruising and tenderness may occur. How long these last and how they affect your daily life varies from person to person. Your medications, dressings and movement instructions are explained according to your operation.

In the early period you will be asked not to put direct, prolonged pressure on the area where fat was transferred. For this reason the patient may be given a special compression garment and a BBL pillow. The BBL pillow helps take pressure off the buttock transfer area when sitting is necessary. However, using the pillow does not mean you can sit for unlimited periods. Your surgeon will tell you how and for how long to use it.

There is no single timeline for returning to sitting, driving, work and exercise. The scope of the operation, the way you heal and your daily life are taken into account. You will also be given individual guidance on lying positions and pressure on the buttocks. Do not use another patient's or clinic's online instructions in place of your own plan.

The compression garment is also part of the post-operative care plan. The type of garment, how long it is worn and which areas it should cover are determined by the liposuction performed. Contact the team if a garment is too tight or causes marked discomfort to your skin.

## Follow-up after you return home

Recovery after surgery is not complete with a visit of a few days to Istanbul. It takes time for the swelling to go down and the body contour to settle. In our clinic we aim to keep online contact with our patients for at least one year. These consultations help us assess the recovery process and answer the patient's questions.

Online follow-up does not always replace an in-person examination. If a new or unexpected symptom appears, the patient may need to get medical care where they are. Before you return, it should be explained whom you will contact and how, what to do in an emergency and when the check-ups will take place.

In addition, all our international patients are insured under the safety umbrella of the health authority in Turkey, and this insurance will continue to protect you even in your own country.

It is also not right to judge the BBL result immediately after surgery. Early swelling affects the appearance; part of the transferred fat may be absorbed over time. The final appearance depends on the patient's tissues and recovery. Some fat loss does not mean the operation has failed; you need to allow enough time before assessing the result.

## What you should know about the risks of BBL

Like every operation, a BBL has risks. Knowing these risks helps you be better prepared when deciding; however, complications should not be thought of as if they will occur in every patient.

The most discussed serious risk in BBL is fat embolism, which can develop when fat reaches the bloodstream. For this reason it is important that the fat is transferred into the appropriate subcutaneous plane, not into the muscle, and that surgical safety recommendations are followed. Real-time ultrasound can help visualise the position of the cannula.

Other possible risks include infection, bleeding, swelling and bruising, absorption of part of the fat, firmness or nodules in the fat tissue, contour irregularities in the liposuction areas and asymmetry between the two sides. The extent to which these apply depends on the person's health and the scope of the operation. The risks and the precautions that can be taken should be discussed openly in the decision consultation.

If after surgery you have shortness of breath, chest pain, fainting, marked swelling in one leg or a symptom that gets unexpectedly worse, get medical help without delay. Keep the emergency contact information you are given at discharge.

## Is our surgery covered by health insurance?

Within the health protection programme for international patients, our clinic will arrange your health insurance and complication insurance before you even arrive in Turkey. This way, from before you board your plane until six months after you return to your country, you will be under the protective umbrella of our national health system.

## What affects the price of a BBL?

The price of a BBL varies with the patient and the surgical plan. The number of areas from which fat is removed and the scope of the procedure, hospital and anaesthesia conditions, and additional services such as accommodation or transfers can affect the overall plan. When getting a quote, it is useful to ask what is included in the price and how post-operative check-ups are arranged.

When comparing different clinics' prices, do not look only at the figure. Find out in which hospital the operation will be performed, how the surgical plan is made, how ultrasound is used, how follow-up is organised and whom you will contact after returning from abroad. Offering affordable, high-quality care without compromising patient safety is our core principle.

## Frequently asked questions

### Is a BBL the same as a Brazilian bottom lift?

Partly, yes. BBL is the abbreviation of *Brazilian Butt Lift* . The logic of the word "lift" here is as follows: when we add volume to a buttock lacking volume with fat injection, the skin covering the buttock starts to stretch, and this has a lifting effect. It is done by transferring fat taken from the person's own body to the buttock area. It is not the same procedure as the operation that lifts the buttock by removing excess skin (gluteal lift, or gluteal autoaugmentation). The operation in which skin is surgically removed and the buttock is pulled upwards is a method used for extremely sagging buttocks, and it is usually preferred when fat injection alone cannot provide a solution.

### Is my own fat used in the operation?

Yes. Fat is removed from your body by liposuction, prepared and transferred to the buttocks. The amount of suitable fat that can be harvested is assessed at the examination. In people with almost no fat, in what is called a "skinny BBL", manufactured allograft fat can also be used.

### Does all of the transferred fat stay?

No. Part of the fat may be absorbed over time. It is not possible to say in advance exactly how much will be permanent. However, because the fat is pooled and made homogeneous before injection, asymmetric absorption or asymmetric loss is generally not expected.

### How long does a BBL take?

It can take about two to three hours. The areas from which fat is removed and the scope of the procedure affect the duration.

### Does ultrasound make the operation risk-free?

Ultrasound helps to see the position of the cannula and the anatomical plane during fat transfer. It can be an important part of safety; however, it cannot be said to eliminate all risks.

### How soon after the operation can I sit?

This depends on the person and the surgical plan. Your surgeon will explain how to use the BBL pillow and when you can return to sitting.

### Can I stay in contact with my doctor after I return home?

Yes. Online follow-up helps maintain contact during recovery. For urgent or serious symptoms you need to get medical care where you are.

## A personal BBL plan in Istanbul

A BBL is an operation that aims to shape the buttock contour using your body's own fat tissue. Whether it is right for you is understood by assessing your expectations, body structure, medical history and usable fat tissue together.

Your process in Istanbul can start with a free online consultation. This is followed by an in-person examination, tests and surgical planning. VIP transfers, accommodation and post-operative check-ups can help you organise your trip. The return date, however, should be confirmed before booking your ticket, or at least together with your surgeon, taking your post-operative condition into account.

In a good BBL plan, the goal is to create a change that is in harmony with your body while also keeping safety and recovery in mind. Asking your questions openly, sharing your expectations and planning the care instructions together are important parts of this journey.
