I first heard this expression from a patient. She asked me to give her almond eyes. When I asked her what she meant by almond eyes and to describe what she wanted from me without the greengrocer's vocabulary, I realised she was describing lateral canthoplasty, a procedure with a history of at least 60 years. Lateral canthoplasty means changing the position of the outer point where the upper and lower parts of our eyelid meet. Let me tell you a little about the aesthetic anatomy of the eye. The lower and upper lids of our eye meet on the inside (medial) and on the outside (lateral). The inner one is even called the tear duct area, because when we cry, tears collect and flow there. The outer one is called the lateral canthus, and in an attractive eye, the line drawn between the lateral canthus and the medial canthus is ideally 4 degrees higher than the horizontal. It can be a few degrees higher still, which can create a sexier look. You know how it is: if someone looks at you with narrowed, attentive eyes, there seems to be an invitation 🙂 So the first question: who should have it? Stand upright in front of a mirror, neither raising nor tilting your head. If the white of your eye is visible below the iris, the coloured part of the eye, between it and the lower eyelid, you should definitely have almond eye surgery, or in classic terms lateral canthoplasty. If your lower eyelid touches your iris, the operation is not necessary, but you can discuss it with your doctor to look sexier. If we have made the right diagnosis, what needs to be done is to move that outer point where the lids meet up by a few millimetres. I prefer to perform the procedure under general anaesthesia. Although many sources say it is very simple and quick, I think it is very important that the lateral canthus is firmly anchored to the bone at the edge of our eye: otherwise your eye may end up even narrower and lower than before. As the Turkish saying goes, don't lose the bead you have at home while trying to get an almond. In short, in a well-selected patient it is a procedure that adds depth and meaning to the gaze.
